The Discharge Co-ordinator will work as a key member of the multidisciplinary team (MDT), supporting the effective coordination of patient flow and discharge processes across inpatient and rehabilitation services. We are looking for an experienced, highly motivatedPaediatric Physiotherapy Assistantto join our friendly paediatric physiotherapy team, delivering services to children with special educational needs in mainstream and special schools, nurseries and the community. The team is part of an integrated Paediatric Therapy Service, providing a multi-disciplinary, family-centred rehabilitation service to children with a variety of complex needs. The successful candidate will support clinical and school staff in delivering physiotherapy to children and young people aged4–19 years with complex health needsacross a variety of community settings. The work is varied and requires a flexible approach, with a focus on promoting good customer service and effective working relationships. The post holder will work as part of thePhysiotherapy Team, supporting Physiotherapists to deliver care to children with physical and movement difficulties and Special Educational Needs (SEN) across Brent.
