EPUT is looking to appoint an enthusiastic Consultant Psychiatrist to join ourHome First team based at Derwent Centre, Harlow, Essex. The Home First Team provides crisis care in the community and sits within the Trust’s Urgent Care Pathway. The team works closely with the three community mental health teams in West Essex based in Epping Forest, Harlow and Uttlesford, who may refer patients who are in a mental health crisis and requiring more assertive management and intensive monitoring in the community. The Home First Team also plays an important role in helping the Trust achieve its Flow and Capacity targets which are in place to ensure effective utilisation of inpatient hospital beds. The Home First Team will “gatekeep” admission to mental health wards with a view to assessing whether the patient could be supported in the community with the Home First Team, thereby avoiding admission. Further, the Home First Team may support inpatient wards in proactively discharging patients who are recovering and now able to be supported in the community. The post-holder is required to work independently and accountably as part of a team of clinicians offering Mental Health services. Working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7 day a week, 24 hours a day service for 365 days a year. You must have full GMC registration and be on the Specialist Register, with an appropriate CCT or equivalent or be within six months of obtaining your CCT on the date of the AAC. Lead on communication support strategies that enhance recovery, reduce distress and improve therapeutic engagement. 7. To manage a complex caseload, communicate agreed treatment/care plans to multi-disciplinary team colleagues and partnership agencies at formal and informal meetings and provide specialist Speech and Language Therapy advice, 8. To apply a variety of specialist treatment modalities to meet the needs of patients, utilising clinical reasoning processes to determine the most appropriate intervention and manage efficacy through outcome measures as appropriate. 9. To delegate and monitor aspects of patient care to assistants, junior staff, students, carers and statutory or non-statutory bodies. 10. To regularly review clients’ progress in order to evaluate and interpret the effectiveness of treatment plans, and amend treatment or discharge clients accordingly. 11. To undertake research and audit to develop and initiate interventions that are evidence based and reflect a sound integration of theoretical knowledge in order to meet patient need. 12. To keep up to date clinical records in line with professional standards and local record keeping policies. 13. To provide written and verbal reports as required at any stage in the treatment process in line with Speech and Language Therapy Clinical Governance Standards. None Clinical: 1. Review and update clinical pathways, protocols and standards of practice aligned with national guidance and local priorities. 8. Actively engage in audits, service evaluations, and quality improvement projects, use outcome measure data to inform service improvements and demonstrate impact. 9. The aim of the service is to provide recovery-focused treatment and support via personalised care plans for individuals who are presenting with a range of mental health disorders: supporting them from diagnosis to recovery, to enable them to lead the lives they wish to live. You will work with a Multidisciplinary Team: a spectrum of clinical professionals, which includes Nurses, Psychologists, Support Time Recovery workers, etc: as well as a range of collaborative system partners. We are looking for a positive team player who has a special interest in working compassionately with individuals and their wider support networks. To work within the MDT and alongside system partners to deliver high quality care and treatment to service users under the care of the specialist community mental health service. To act as a Care Co-ordinator; formulating care plans, completing/updating risk assessments and collaboratively review care needs via the CPA approach. To work alongside inpatient services; supporting timely discharges from hospital in keeping with policy and processes. To fulfil the role of the Duty Worker as part of a service rota.
