The post holder carries responsibility for the assessment of care needs and their development, implementation and evaluation of programmes of care for Adults and Older Adults with mental health problems that are referred to the Crisis Resolution Home Treatment Team. The post holder is responsible on a day to day basis to the Clinical Lead but ultimately accountable to the Matron for the overall service. In addition, they will be a member of a multi-disciplinary team and work in close co-operation with wards and Primary Health Care based professionals. They will also be responsible for the maintenance of standards of care within the Crisis Resolution Home Treatment service. To advise Line Manager and Professional Social Work Supervisor and/or Nurse Advisor on all breaches of Nursing/Social Work/Occupational Therapy professional conduct as required by the NMCs/HCPC Code of Professional Conduct. 11. In this varied role, you will support the cleansing, validation and correction of waiting list data across a range of specialties, including diagnostics, planned care and inpatient pathways. You will also help ensure outstanding outpatient appointments are cashed up in a timely manner, supporting the Trust to maintain accurate information and effective patient pathway management. Your professionalism, accuracy and ability to work across a range of Trust systems will be essential. You will liaise with divisional teams, service managers, clinical colleagues, booking teams and patient access colleagues from across Broomfield Hospital to resolve queries, support consistent data quality and escalate issues where required. We are looking for someone who is enthusiastic, highly organised and confident using computer systems, with strong communication skills and a clear understanding of the importance of confidentiality. You will need excellent attention to detail, the ability to manage your own workload and the flexibility to respond to changing priorities. Previous experience in an administrative role would be beneficial.
