This role has been developed in order to improve the patient experience by providing key worker contact for patients and by delivering nurse-led clinics to ensure timely assessment and follow up. It will form a key part of the Rhinology patient pathway, working alongside two Rhinology ENT consultant colleagues and collaboratively with the wider multi-disciplinary team. We are looking for an experienced ENT nurse who is able to demonstrate expert clinical skills in order to provide safe, effective and efficient patient care with the guidance of Consultant Rhinologists. Prospective candidates must be able to evidence meeting the four pillars of nursing in order to demonstrate their ability to meet advanced nursing roles but in house role specific training in rhinology will be provided to the successful candidate. This will improve clinic capacity and current standards of care. The post holder will deliver high quality specialist nursing care, including key worker support for high-risk pathways, whilst acting as a resource to the Rhinology team and by providing bespoke education to both patients and to staff. You will support patient assessment by both requesting and actioning the results of appropriate imaging and tests, provide expert guidance to complex patients to mitigate against emergency admissions and facilitate discharge, advise patients on medical optimization for surgery and provide co-ordination with local hospitals around the region to help provide care closer to home where clinically appropriate. The post holder will actively engage in quality improvement work to support service improvement and will participate and lead on both clinical audit and research within the department in order to improve patient outcomes.     The Band 6 Hospital Liaison Practitioner provides specialist mental health assessments, risk management and short-term interventions for patients presenting to the Emergency Department and acute hospital wards. Working within a multidisciplinary Hospital Liaison Team, the post holder delivers timely mental health care, supports crisis management, and coordinates care with acute hospital staff, community services and partner agencies. The role requires autonomous decision-making, management of complex cases, and contribution to the delivery of a safe, responsive and recovery-focused liaison mental health service. • Undertake comprehensive mental health, psychosocial and risk assessments for patients presenting to the Emergency Department and acute hospital wards. • Provide timely assessment, intervention and care planning for individuals experiencing mental health crises within the acute hospital setting. • Manage a caseload of patients, coordinating care and ensuring effective referral, discharge and follow-up arrangements. • Deliver specialist mental health advice, support and consultation to acute hospital staff and multidisciplinary teams. • Assess and manage clinical risk, including safeguarding concerns, and implement appropriate risk management plans. • Work collaboratively with community mental health services, primary care, social care and third-sector organisations to ensure seamless patient care. • Facilitate admission avoidance where appropriate and support timely discharge planning from acute hospital services. • Maintain accurate clinical records, documentation and reports in accordance with Trust policies and professional standards. • Participate in clinical supervision, audit, training, service development and quality improvement activities. • Contribute to the achievement of service performance targets and response standards within the Hospital Liaison Service. This is an exciting opportunity for a consultant with expertise in GI pathology, Gynaecological pathology, non-gynaecological cytology, Bowel Cancer Screening Programme reporting and cervical screening histology to join a large, supportive team of subspecialist pathologists. The successful candidate will play a key role in diagnostic reporting, MDT participation, supporting clinical decision-making and patient care, service development and quality improvement activities, while acting as Deputy MDT Lead for GI and Gynaecological services. You will contribute to clinical governance, audit, quality assurance, teaching and service development, while working collaboratively with a large team of consultant pathologists to support innovation, including the implementation of digital pathology and expansion of molecular diagnostic services. The post offers flexible and remote working opportunities, with no out-of-hours commitment and no autopsy requirement, providing an excellent balance between professional development and work-life flexibility.
