    * The banding for this vacancy is subject to Agenda for Change outcome * The key role is to lead the United Kingdom Accreditation Services (UKAS) Quality Assurance Programme - Improving Quality in Physiological Sciences (IQIPS) for the Audiology Service. This is an NHS approved accreditation programme designed to support healthcare organisations ensure patients receive consistently high-quality care, measured and assessed against National standards. The post will provide a range of project management, improvement methodology, data processing and analysis to ensure the service attains and retains accreditation. The post holder will keep up to date with UKAS requirements and standards, collect, collate and submit quality and audit data, support progression through the accreditation process and facilitate change within the audiology service, based on UKAS assessment findings and standards. The post holder will lead in the Quality Assessment Inspection of the Service in the national yearly assessment. The Audiology Quality Manager will work across The Shrewsbury and Telford NHS Trust and community outreach venues, providinga key role in supporting the Head of Audiology in maintaining a Quality Management System. • This role helps make sure the Audiology Service provides safe, high-quality care that meets national standards. • The post holder leads and supports the service's UKAS accreditation programme, helping the team achieve and maintain accreditation. • They collect, check and analyse data, audits and performance information to monitor quality and identify areas for improvement. • The role works closely with Audiology managers and staff to update policies, improve processes and support quality improvement projects. • They prepare reports, maintain quality management systems and provide information for inspections, assessments and national reviews. • The post holder also helps ensure equipment, training, records and service performance meet required standards across all Audiology sites.     This is an exciting opportunity to join the patient safety and governance team at Hampshire Hospitals NHS Foundation Trust as a patient safety incident investigator. The implementation of the NHS Patient Safety Incident Response Framework has led to the introduction of a patient safety incident investigators who are responsible for the investigation of patient safety incidents across all three sites – Basingstoke and North Hampshire Hospital, Royal Hampshire County Hospital and Andover War Memorial Hospital. Investigations are undertaken in line with the Trust patient safety incident response plan and use a systems-based approach. The patient safety incident investigator will support the head of patient safety and risk to provide Trust-wide oversight, assuring the quality and effectiveness of all types of patient safety incident investigations as part of the Trust’s annually agreed Patient Safety Incident Response Plan (PSIRP). This is required within the Patient Safety Incident Response Framework (PSIRF) which has been implemented in response the NHS Patient Safety Strategy. The patient safety incident investigator will undertake robust patient safety incident investigations by working with a range of internal and external stakeholders to investigate, analyse and use judgement through the collation of evidence and findings using the agreed methods within the Trust’s PSIRP. The outcome will be the identification of systems learning from a patient safety incident that will then guide sustainable risk reduction and service improvement, ensuring that a Fair and Learning Culture and human factors principles are maintained.
