    The Band 7 Mental Health Liaison Team (MHLT) Bank Practitioner provides specialist mental health assessment, risk management and intervention for patients presenting to the Emergency Department (ED) and Acute Medical Unit (AMU). Working as part of a multidisciplinary team, the post holder will deliver high-quality, patient-centered mental health care to individuals experiencing acute mental illness, self-harm, suicidal ideation, behavioural disturbance, substance misuse, dementia, delirium and complex psychosocial crises. As a senior clinician within the Mental Health Liaison Team, the post holder will provide expert mental health assessment, formulation and care planning for patients with complex presentations. They will work collaboratively with acute hospital colleagues and external partners to ensure patients receive timely assessment, appropriate treatment and onward referral to the most suitable services. The post holder will support the day-to-day operational delivery of the liaison service during bank shifts, providing clinical leadership, advice and guidance to colleagues where required, whilst maintaining high standards of clinical practice, documentation and risk management.     This is an exciting opportunity to join the Forensic Outreach Service in one of the dedicated clinical posts supporting a new forensic step-down supported living service, scheduled to open in January 2027. The Service will be delivered as part of the SWLSTG Adult Secure discharge and step-down pathway, commissioned through the South London Partnership (SLP) Adult Secure Provider Collaborative Programme. The service will provide an integrated model of care and support through a partnership between the Forensic Outreach Service and a Voluntary and Community Sector (VCS) provider. The VCS partner will deliver the supported living pathway and work closely with the SWLSTG Forensic Outreach Service which will host dedicated clinical posts. The Service is commissioned for adults stepping down from Adult Secure inpatient services into a community setting, whose ordinary residence and Section 117 aftercare provision sits with one of the five South West London and St George’s Mental Health NHS Trust (SWLSTG) boroughs, for health and social care. It will enable people to transition safely and sustainably from secure mental health services into the community, through access to high quality accommodation and specialist, recovery-focused support delivered in a least restrictive setting. The successful candidate will work within the Forensic Directorate in providing Occupational Therapy input to individuals living within the Forensic Step-Down Service. The post holder will sit within the Forensic Outreach Service. The post holder will play a key role within the Forensic Step-Down Accommodation Service, supporting individuals with complex mental health needs and forensic histories to successfully transition from secure or inpatient settings to independent community living. The post holder will work with children, young people and parents with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities. The post holder will also be required to complete a 1 year EMHP supervisor training programme if not already completed The MHST works with Young people with mild to moderate mental health presentations attending school/education setting in the Harrow area. The purpose of the team is to • Provide evidence-based 1-1 and group interventions to CYP and parents for mild to moderate mental health issues. • Support the senior mental health lead in each education setting to introduce or develop their whole school or college approach. • Give timely advice to school and college staff, and liaising with external specialist services, to help children and young people to get the right support and stay in education. Young people have access to the MHST through their education setting. School staff and other potential referrers are able to contact a team of clinical staff to discuss potential referrals. The MHST clinician will manage referrals, deal with queries, undertake clinical telephone triage and face to face assessment, give advice and signpost young people where appropriate. The role involves regular supervision of junior clinicians within the team including Trainee and Qualified Education Mental Health Practitioners.     Senior Staff Nurse - Intensive Care, Band 6 The Intensive Care Unit comprise of a combination of Level two and Level Intensive Care beds. We care for a broad range of critically ill patients and offer a friendly and supportive working environment were education is seen as a priority. If you join us we will expect you to provide excellent, compassionate care to all our critically ill patients and their families. We are looking for nurses with experience in intensive care: You will have completed your intensive care course, have experience of supporting learners and be keen to pursue your career in intensive care. The band 6 role focuses upon the development of nurses, involvement in project work before moving on to develop skills in coordinating of our general intensive care unit and Burns Intensive Care. You will be given a structured education plan to meet your individual needs to help you to continue to develop as an intensive care nurse. It is an exciting time to join the team at Chelsea and Westminster Hospital as we have just completed our refurbishment in our ICU area for increased capacity. The post holder will be a clearly identifiable senior clinical staff member, and team leader. They will have significant clinical experience, provide sound clinical expertise to their team, and coordinate and deliver high quality care. They will be a role model for expert clinical care, and lead on specific projects / management functions as delegated by the Sister / Charge Nurse. They will role model the identified Trust values at all times. In general settings, they will be the assigned deputy for their manager, generally known as a Junior Sister / Charge Nurse, coordinating clinical, managerial and education requirements of the area, and being a role model for team members, whilst delivering and overseeing high standards of care. In midwifery settings they will be called a Midwife. In specialist settings, they will have a significant level of specialist clinical knowledge and expertise in the clinical functions of care delivery. They will provide advice and guidance also, and generally be known as a senior staff nurse. Please note:Some roles may require weekend shifts at multiple sites. Leadership and management: • Manage the day to day staffing arrangements and rotas, to ensure safe levels of practice • Be a shift coordinator on a regular basis where appropriate • Ensure staff adhere to Trust protocols of care, eg Infection Control • Take continuing responsibility for the Ward / Department in absence of manager • Participate in senior nurse / unit on call rota / bleep holder (some areas, eg paediatrics) as appropriate • Lead and manage their own clinical team within general clinical areas, and of the total team in the absence of the manager, in line with Trust values • Develop and maintain effective communication and working relationships with other professional groups, and staff generally within the Division and the Trust • Contribute to the effective management of the area by ensuring correct levels of staff are in place to deliver and assist with the planning of off duty rotas • Assist in the delivery of induction / orientation of new staff • Act as a role model, ensuring staff work to Trust guidelines and values • Supervise, under the direction of their manager, the work of qualified and unqualified staff and learners • Provide clinical support and guidance to staff • Act as a resource for clinical knowledge to staff • Initiate action on complaints & incident management and responses, and assist in their investigation and resolution • Participate in recruitment and selection, and develop effective selection and interviewing skills • Undertake appraisals of staff, following training • Chair team meetings as required • Lead on specific projects, as indicated and agreed by manager • Have a working knowledge of budget arrangements for area, focussing on cost effectiveness and quality, to ensure maximum use of resource • Understand and manage local change with team members, including the embedding of new projects 3.Education & research: • Participate in the education of all learners within their area • Actively support and assist in preceptor ship and in pre-registration student support, supervision and guidance including mentorship where appropriate • Identify and respond to learning needs for area • Develop and deliver or organise local training sessions, mindful of the Trust values • Facilitate others in the development of informal teaching skills • Participate in the annual training needs analysis for their area • Participate in any local or related research projects, clinical trials etc • Apply and encourage others to utilise evidence based practice and relevant research findings in practice • Identify areas of nursing / midwifery research within their area that will be of benefit, and encourage and educate other staff to recognise the importance of research • Participate in clinical supervision / acting as supervisor 1.
