This is an exciting opportunity to play a key role in a major transformation of urgent and crisis mental health services acrossKingston, Merton, Richmond, Sutton and Wandsworth. As a senior clinician within a new front-door service, you will be at the centre of crisis decision-making, ensuring people receive the right support, from the right service, at the right time. You will work closely with a wide range of partners across mental health services, NHS 111, ambulance services, acute hospitals, primary care, social care and VCSE organisations to provide expert clinical guidance and facilitate seamless access to care. The Crisis Access Hub providestelephone and digital screening, triage, prioritisation and onward routing. While the service does not deliver face-to-face assessments, home visits or treatment, you will undertake comprehensive clinical reviews and, where referral information is insufficient, leadenhanced triagesthrough telephone or digital contact with patients, carers and professionals. This will enable you to gather additional information, clarify presentation and risk, and make informed decisions about the most appropriate pathway of care. In this senior role, you will provide expert clinical decision-making, support and supervision to Band 6 Clinical Practitioners, coordinate referral flow during shifts, and play a key part in ensuring the service achieves its ambitious1-hour screening target. You will also contribute to service development, quality improvement, audit, and the ongoing evolution of the service.     Clinical Fellow (ST5-7 LEVEL) in RAC We are seeking an outstanding clinical Fellow in Rapid Access. The post is at Hillingdon NHS Trust which has an excellent reputation for its friendly, supportive culture. The post is suitable for an ST5 or above doctor in Obstetrics and Gynaecology. The post is appropriate for those wishing to gain out of programme experience (OOPE). It may also be suitable for those not currently in training or those who have finished training. The service sees approximately 1500 patients per year and last year diagnosed 93 cancers. Our aim is to further improve our FDS target and streamline services to introduce more one stop clinics and straight to test diagnostics. The timetable will consist of rapid access clinics, outpatient hysteroscopy (diagnostic and operative) and colposcopy. These will initially be supervised with the aim of working towards independent practice. There will be an alternate week theatre session. The post will include daily ward rounds of any gynaecological cancer patients, prepping for the local MDT meeting and virtual attendance at the central MDT meeting at Hammersmith. In addition, the role will include: - Gynaecology outpatient clinic - Gynaecology on-call session - Guideline writing, audit and research - Teaching and supervision of other trainees (once appropriate) There will be some flexibility in tailoring the timetable and goals to the experience and wishes of the individual appointed and will support those wishing to work towards RCOG intermediate laparoscopy and ATSM’s in Oncology, benign gynaecology surgery: hysteroscopy and benign abdominal surgery: open and laparoscopic. There is also the potential to attain BSCCP colposcopy accreditation. Training will be supervised by Mr Neale Watson (Rapid Access Lead) in conjunction with the other rapid access Consultants (Mr Maheswari, Mr Chilcott, Miss Pugh and Miss Touqmatchi) as well as Mr Ayim (for hysteroscopy) and Miss Kothari (colposcopy). The postholder will also take part in the regular out of hours Gynaecology and Obstetrics SPR rota (but 80% of daytime work will be fellowship related, as outlined in the timetable below). We deliver acute services at Hillingdon Hospital, as well as some services at Mount Vernon Hospital. The Trust has a turnover of around £250m and employs over 3,300 staff. We deliver high-quality healthcare to the residents of the London Borough of Hillingdon, and increasingly to those living in the surrounding areas of Ealing, Harrow, Buckinghamshire and Hertfordshire, giving us a total catchment population of over 350,000.     We are looking for an enthusiastic, motivated and hardworking surgeon for the position of Post-CCT Fellow in the Specialist Hip & Knee Unit. We are recruiting for: 1 x 4 month post Dec 2026 - March 2027 A curriculum vitae will be required with the application form. The Specialist Hip & Knee Unit offers an exceptional fellowship opportunity for surgeons seeking advanced training in orthopaedic procedures. Led by a team of 12 substantive consultants, the fellowship provides comprehensive exposure to complex hip and knee arthroplasty, with unique flexibility for candidates to tailor their experience towards either arthroplasty, hip surgery or knee surgery. The Fellow’s post will provide exposure to all aspects of the management of patients with primary and revision hip and knee arthroplasty and complex limb reconstruction. There will be ample opportunity to assist and gain experience with the full range of extremity and pelvic surgical cases. Attendance at arthroplasty MDT meetings is expected. The Fellow will be allocated to one of the sub teams within the unit and have the opportunity to rotate at 6 months. The Fellow will attend fixed outpatient clinics and operating lists. There will be opportunities to sit in clinic with the consultants as a learning experience as well as see patients independently. Likewise, in theatre, there will be opportunities to assist with complex surgical procedures as well as undertake surgery as the lead surgeon. As the fellow gains in experience they will be able to work more and more independently over time. There will be supervised research sessions. The post holder will be expected to present research at national and international meetings. The Royal National Orthopaedic Hospital NHS Trust was formed in 1991 as part of the first wave of NHS Trusts.
