Our expanding community service covers patient in their own home, community hospital sites and also in a care home setting. Our care home pathway utilises digital healthcare to support with achieving timely assessments for patients. Applications are invited for 2 posts of Senior Clinical Fellow in Pancreatiobiliary Medicine within the Royal Free Hospital Liver Unit, a liver transplant and tertiary HPB surgical referral centre with a large pancreatobiliary endoscopy service. The successful candidates will contribute to the clinical care of patients with a wide range of hepatobiliary disorders through the triage and co-ordination of pancreatobiliary endoscopy referrals, including pre and post procedure management. The post-holders will work closely alongside HPB surgery and hepatology teams, as well as liaise with external referrers. There is the opportunity to attend up to 11 pancreatobiliary training endoscopy lists per week, performing ERCP and EUS under consultant supervision. The posts also offers experience in a range of other fluoroscopy-guided interventional endoscopy procedures, including enteral and colonic stent placement and jejunal feeding tube insertion. The appointees will be responsible for co-ordination of the weekly benign joint Royal Free/UCLH HPB multi-disciplinary team meeting. Depending upon their training needs and interests, there is the opportunity to gain further experience through attendance at weekly MDTs in liver transplantation, hepatobiliary malignancies and hepatology radiology and pathology meetings. There is no formal inpatient commitment other than review of patients referred for endoscopic interventions. Attendance at weekly specialist clinics in pancreatobiliary medicine and autoimmune liver diseases is encouraged. The posts includes a contribution to the out-of-hours rota for hepatology and liver transplantation. The Unit has an excellent research reputation and an interest in research is desirable with suitable projects available depending on the candidate’s interest. The Pancreatobiliary Medicine service complements the HPB surgical and liver transplant practices. The unit has recently expanded to a total of 11 regular pancreatobiliary endoscopy lists per week (five with anaesthetic support) and currently undertakes more than 700 ERCP and 600 EUS procedures annually, providing excellent opportunities for clinical training. We have an established cholangioscopy service as well as offering interventional EUS procedures including pancreatic cyst and biliary/gallbladder drainage procedures,Endoscopic ultrasound-directed transgastric ERCP(EDGE) as well as EUS guided gastro jejunostomy Hepatology and Hepatobiliary Medicine at the Royal Free Hospital The Royal Free provides a wide range of secondary and tertiary Hepatology and HPB services. There is excellent access to ITU, a 24 hour TIPS rota and 24-hour consultant led GI bleeding rota. The liver transplant service has been established for 25 years and the number of transplants performed has recently increased recently, currently standing at approximately 125 per year. We have a live donor program and are seeking to expand this activity. We are a tertiary centre for patients with hepatobiliary and pancreatic malignancy and the full range of surgical, locoregional and oncological treatments is provided for these patients. Dedicated clinics are established for viral hepatitis, liver metabolic disease, primary hepatocellular malignancy, autoimmune liver disease, PBC, PSC, NAFLD, Vascular liver disease as well as clinics for general hepatology and liver transplantation. Applicants are invited to apply for the post of Senior Locally Employed Doctor (ST3+ Equivalent) in Reproductive Medicine. The post is suitable for doctors wishing to gain clinical experience in Reproductive Medicine, either to pursue a career in this subspecialty or to complement their career in general gynaecology. These posts are designed to provide invaluable experience in this subspecialist area whilst providing clinical services in the Department of Reproductive Medicine, working closely with the Consultants and multidisciplinary team. Candidates that are non-GMC registered at the point of application will be considered for this position, under the Trusts International Fellowship Scheme (please note, there is a £500 admin charge for this scheme). Please note there are 2 posts available- one full time and the other 30 hours. • Outpatient clinics:The clinical fellow will provide care for couples presenting with subfertility, including those needing assisted conception treatment. They will conduct initial fertility assessments, identify risk factors, adequately counsel and arrange further investigations as required. The clinical fellow will work under supervision of a consultant and recognise their roles within the multidisciplinary team • Ultrasound scan lists:The clinical fellow will be trained to undertake independent gynaecology and fertility scans lists. There is the opportunity to complete British Fertility Society certification for fertility scanning. • IVF procedure and assisted conception skills:The clinical fellow will gain experience in managing couples with infertility and those undergoing assisted conception. They will gain training in egg retrievals, intrauterine inseminations and embryo transfers. Their responsibilities will also include review of patients admitted to the ward with complications of treatment (such as OHSS). • Administrative work:The post holder, in collaboration with the other Clinical Fellows already employed in the department would have responsibility for the day-to-day management of patients undergoing assisted conception treatment. The Endoscopy department at University Hospitals of Derby & Burton NHS Foundation Trust currently have 3 endoscopy units across 3 of our sites: • Royal Derby Hospital - 7 Rooms • Queens Hospital Burton - 3 rooms • Sir Robert Peel - 2 Rooms And delivery over 30,000 procedures annually, all sites are currently JAG accredited. We have a dedicated Clinical Lead for endoscopy along with a Clinical training lead. Endoscopy at RDH perform ERCP, EUS, capsule endoscopy and complex EMR procedures. We have a dedicated ERCP procedure room, provide an emergency inpatient endoscopy list 365 days a year and have a dedicated endoscopy nurse on call rota These exciting newly appointed roles will include back filling cancelled endoscopy sessions (5 per week along with 2 training sessions) but we will also encourage specialised training for endoscopic procedures such as complicated EMRs and EUS. The University Hospitals of Derby & Burton Gastroenterology department have: • 15 Luminal Consultants • 6 Hepatology Consultants • Our surgical colleagues' also support the unit: • 10 Colorectal surgeons • 9 Upper GI surgeons • 10 Consultant surgeons at QHB Due to existing endoscopy clinical fellows training requirements, specialist training can only currently be delivered in HPB endoscopy, i.e. ERCP and Endoscopic ultrasound. This post will be based at the Royal Derby Hospital campus and is designed to support the activities of the endoscopy department across all 3 sites and acquire specialist advanced endoscopy training. The post-holder will work alongside a variety of Consultant Gastroenterologists and GI Surgeons and will be responsible to the clinical lead in Endoscopy. This is a one-year post specifically intended for registrars (or equivalent) approaching their CCST or immediate post CCST (or equivalent), and wishing to expand their endoscopy / research experience and undertake further training in therapeutic and advanced colonoscopy, ERCP, EUS and as such is most suited to those reaching the end of their specialist training in gastroenterology. As the candidate will be conducting service endoscopy sessions independently they will need to have achieved competency in diagnostic upper and lower GI endoscopy. Experience in capsule endoscopy reporting and common therapeutic endoscopic procedures is also desirable but not essential. It would also be suitable however for anyone with sufficient endoscopic experience e.g. staff grades in gastroenterology. The appointee will be expected to undertake 5 service endoscopy lists and 2 training lists in either ERCP, EUS, or advanced colonic polypectomy (EMR). Appointees will be expected to identify a training requirement at interview (i.e. undertake 12 months training in either ERCP, EUS, or advanced colonic polypectomy (EMR).
