He's palliative end of life and his wish is to receive his care at home with his family. The gentleman lives with his wife and 13‑year‑old daughter. He also has three grown-up children. He has a diagnosis of Adrenoleukodystrophy (ALD), a rare inherited disorder affecting the brain, causing a progressive loss of physical and mental skills. As a result, he has young onset dementia impacting all aspects of his daily life, along with epilepsy and he is steroid dependent. About the Role This is now a 24/7 care package, and we are specifically recruiting for maternity cover for the following shifts: Monday: 8:00am – 9:00pm Tuesday: 8:00am – 9:00pm Wednesday: 8:00am – 9:00pm Thursday: 7:00am – 9:00pm The maternity cover is likely to commence October time, maybe earlier depending on situation. Additional hours may be available, including overnight shifts starting 9:00pm until 7:00am, which can be discussed at interview. Key Responsibilities The successful applicant will: The gentleman does not have anything orally, he does enjoy mouth care with swabs which is very important. He has a peg fitted and has feed and all medication administered this way. He requires full-time care, you will be expected to carryout all personal duties such wash, dress, shave etc. He is fully incontinent, so cleanliness and skin integrity is very important. Administer medication Support with moving and handlingis essential aspect of his day, he cannot control/move his own body. He uses a ceiling hoist, has a adapted bed and chair that he uses. Also an electric wheelchair which is carer controlled. Build a positive relationship through encouragement, engagement, and communication. He is non verbal but he listens to sounds and finds this comforting. Support him in caring for his daughter when his wife is at work Work alongside his family and wider care team to meet his needs and access appropriate activities. He is full of laughter and mischief and is very gregarious and friendly. He loves being outside, car/bus journeys,animals, driving his powerchair, water, music, baking, arts/crafts and people watching! When possible he likes sailing, kayaking, horse-riding and climbing! He needs help with all of these passions because of his cerebral palsy. This role is very flexible, and can be shaped to a large extent by your availability, interests and preferences. Basically we are looking for ALL, or ANY, of: * Evening care - once or twice a week, between about 4pm-8.30pm. This would include feeding, bath and bedtime routines at home, but could also involve some entertainment, play, trips out and about etc * Daytime care on some weekends and in school holidays - anything from a couple of hours up to a full day; we are flexible and will work with you to find an arrangement that suits you and Finn. This would most likely include taking Finn out and about on trips / fun activities, driving him in his wheelchair-adapted van. * After a period of you and Finn getting to know each other and building confidence, there could be the option of fully paid respite care periods of 24-48 hours (or potentially even more). Most likely this would be at Finn's home with the rest of his family away. We will train you in personal care, tube feeding via gastric button, hoist transfers, positioning in wheelchairs, supporting Finn to drive himself in his powerchair and communicate using his eye gaze computer. Pay rates start at £15/hr but this will increase once you have gained a reasonable level of experience (usually takes a few months). A driving licence is essential, but no previous experience is necessary. Full training is provided. The bungalow is fully adapted for my needs with a wet room and track hoists. I like to keep independent with decision making and choices. We often have family and friends visit, but everyone is extremely welcoming and kind. I live with my wife and 8-year-old stepson. I hope the person who gets the role will become a lifelong friend as well as PA. At night I use a ventilator, and I use a regular nebuliser/cough assist machine. We have four cats. The PAs have use of their own room. You’ll be supporting me with day-to-day living, including (but not limited to); 5 hours of work spread through the day and night. Prompts and support with eating, drinking, and medication. Meal prep/cooking (incl. batch cooking). Cleaning e.g. laundry, bins, dishes, CPAP utilities. Support with organising/decluttering. Some night waking - I tend to go to bed in the early hours of the morning (1.30 - 3 am) and wake in the afternoon (1pm - 2pm).
