We are looking for a motivated registered nurse with experience in breast oncology care to join our oncology breast service . The post holder will be expected to work autonomously as part of a team. This role is part time, 22.5 hours per week for a fixed term contract of 23 months. The successful candidate will be expected to develop and implement a new endocrine therapy support service. In addition, the successful candidate will also be expected to play an active role in designing and refining care pathways, develop standard operating procedures, create patient information resources, and establish collaborative working processes between primary and secondary care. They will help identify service needs, analyse patient experience and activity data, and contribute to continuous quality improvement initiatives that strengthen the endocrine survivorship pathway. • The Nurse Practitioner for Breast Oncology will provide specialistnursing support to people with early breast cancer who are receiving endocrine therapy and experiencing treatment‑related side effects, including therapy‑induced menopause. The team is a trust wide service covering all five boroughs of Pennine Care and it is essential that you drive and have access to transport to travel across the whole footprint of the trust. We are currently based at Trust HQ in Ashton Under Lyne. Our core hours are Monday to Friday 9am-5pm. To embed the Occupational Therapy assessment and intervention pathway across the Community Rehabilitation Team, within the framework of Model of Human Occupation in collaboration with the wider MDT. To be responsible for a caseload of service users under the care of the Community Rehabilitation Team, supporting them in their recovery journey. To undertake specialist OT assessments in all areas of self-care, productivity and leisure to support discharge planning and identify support needs. To conduct OT assessments, developing outcome-based needs led care plans, facilitating the implementation of the agreed plan of care and evaluating its effectiveness utilising evidence-based outcome measures. To provide a range of highly skilled interventions that will be delivered on both a group and individual basis and share those skills with other team members. To develop treatment plans and intervention to be delivered by the team’s social inclusion workers. To provide clinical supervision to the junior staff within the Community Rehabilitation Team. This may include people who may have difficulties with understanding or communication. • To be responsible for managing a caseload of clients as required. • To manage patient discharge in a safe and timely manner, ensuring liaison with relevant professionals, to ensure that family and carers are, where appropriate, advised of the patients discharge arrangements, with timely completion of documentation. • To develop partnership working with all relevant agencies/services/individuals to support optimal care for patients • To work in partnership, direct, support and supervise the team’s technical instructor.
