About the Role As the Extra Care Scheme Team Leader, you will play a key role in the day-to-day operation of the scheme, ensuring that our residents receive safe, responsive, and person-centred care. You will provide leadership, guidance, and support to the care team while working closely with the Registered Manager and other professionals to maintain the highest standards of care. Key Responsibilities - Lead, motivate, and support a team of Care and Support Workers. - Ensure high-quality, person-centred care is delivered in line with individual care plans. - Coordinate daily care delivery and allocate workloads effectively. - Carry out staff supervisions, observations, and contribute to performance management. - Support the induction, mentoring, and development of new team members. - Monitor care documentation to ensure accuracy and compliance. - Promote safeguarding, dignity, choice, and independence for all residents. - Respond appropriately to incidents, emergencies, and changing care needs. - Liaise professionally with families, healthcare professionals, and external agencies. - Assist with audits, quality assurance, and continuous service improvement. - Participate in the on-call rota where required. - Lead by example by delivering hands-on care when needed.
