Our home provide both short and long-term care and support to Children and Young People who are unable to live within their own family home. About the role: Our solo provisions predominantly involve those on shift caring for children who may display a range of emotional, social or behavioural difficulties, including children who may have experienced trauma, have attachment difficulties, children showing sexually problematic behaviour or at risk of sexual exploitation. They may be struggling with mental health issues and have suicidal tendencies, require specific support from our provision, or be subject to a depravation of liberty order by the courts and require a specific reduction plan to support them. Children who have experienced substance misuse or children experiencing relationship difficulties within their families and supportive networks; or young people with ASD may also require our service. Our solo provisions are situated within Hulls local community and designed not to stand out as different to anyone else’s family home. The solo provisions are small, but comfortable and caring, creating a warm environment for those that come to reside at their new home. Our shifts within the Solo provisions consist of a one day on/ two days off pattern – start times will vary dependant upon the manager of each solo provision. The role is to provide a highly skilled level of care to each unique young person in our homes. This will include taking the lead on shift as there are only two staff members per shift, you will be expected to work within our lone working policy and make professional safeguarding decisions that will impact both the young person and the team – this will specifically apply where there is no senior care officer present. You will also be expected to be competent in both written work and admin that is specific to the role and be able to clearly communicate any information necessary, or that needs passing on to relevant professionals. You will be expected to work proactively and positively with our complex young people in what can be a challenging and demanding role. The solo provisions provide a rewarding environment in which working with our young people will provide a platform to help them achieve their full potential and move on positively into the next chapter of their life. About you: We require all Care Officers to have: A level 3 Diploma in Residential Childcare or equivalent, or the ability to complete this within two years of appointment. Experience working with vulnerable adults or children in a safeguarding or care setting. A strong motivation to work with Children and Young People and ability to form and maintain appropriate relationships. An ability to work within relevant framework when assessing situations in order to make decisions and recommendations. You must be experienced, skilled and confident to lead on all elements of care, and be able to guide and support others. Hours per week will vary, totalling 74 per month and will include long shifts and sleep in shifts. Hours will exceed 20 hours per week due to the rota. ROLE AND PURPOSE: To provide a range of support to children and young people who are unable to live within their own family and require short term or long term residential care. In line with relevant legislation and corporate parenting levels of responsibility. PRINCIPAL ACCOUNTABILITIES:1. To promote and safeguard the welfare of children, young people and/or vulnerable adults. Within the context of individual care plans or instructions from the Line Manager to assess the needs of children in need and children looked after in order to provide care which is responsive to their individual needs. 2. Statutory Obligations – As per statutory requirements as specified in the Children’s Homes Regulations and Quality Standards. 3. To use systemic practices to prepare, implement and review effective programmes for individual children and families/carers to ensure that their needs for development and/or independence are met. Work in partnership to assess needs and in consultation with Senior Care Officers, Children’s Home Manager, Social Workers and other Managers, plan appropriate packages of support. 4. Under delegation/supervision assume responsibility for specific issues i.e. for the health and safety of colleagues and children by identifying areas for improvement and responding to deficiencies in the workplace to ensure a safe working environment or for Quality Assurance to enhance the performance and effectiveness of service delivery. With a requirement to have the ability to stay calm when faced with confrontational situations. 5. Acts as a nominated ‘Shift Leader’ as required responding to the need of the service. 6. Provides an efficient and responsive service to children for their admission into and discharge from residential care to ensure their needs are met during these transition periods. 7. Assist children with a range of tasks including primary care and, where necessary, carries out tasks on their behalf whilst at the same time promoting independence within individual’s capabilities. 8. Acts as a designated worker in respect of individual children, including attending meetings/case conferences and dispensing medication as required, to ensure their safety, well being and quality of life. 9. Contributes to an efficient administrative system by preparing information for the Children’s Home Manager and quality assurance systems to ensure effective communication. Provides accurate reports and where appropriate, assessments to inform packages of support and care with special consideration to the transition period of admission and discharge. 10. Participates in budgetary tasks i.e. pocket money, purchasing of groceries, clothing, stationery etc, to ensure value for money within budget provisions. 11. Assist children to deal with complex needs or particular vulnerabilities to ensure their physical and emotional well being which will include implementing Therapeutic Crisis Intervention techniques. 12. To work and liaise with voluntary services, statutory organisations and colleagues within other parts of the Local Authority with the focus of providing up to date knowledge and working practices for intervention and service delivery to children in need and their families. 13. Works flexibly to meet the assessed needs of children and their families, by offering support to families and carers, sometimes in their own homes. Assist in providing day care and outreach as required, and work in partnership with schools and alternative educational provision to support and maintain an educational placement. 14. Refers to Line Management upon any concerns, suspicion, evidence or allegation in relation to child protection in accordance with the Safeguarding Board guidelines to ensure the safety and welfare of the children. 15. Intensive care medicine and anaesthesia fall within the surgical care group specifically within the anaesthetic department at Darlington Memorial Hosptal. Complimetary anaesthetic sub-specialities including Chronic Pain, Acute Pain, Pre-assessment, Day Surgery and obsterics. The care group director for Anaesthetics, Theatres and Intensive Care Medicine is Dr James Limb (trust wide). The lead clinician for the anaesthetic department at Darlington Memorial Hospital is Dr Jothika Thimappa and the lead clinician for the Intensive Care department is Dr Simone Carbert. The successful applicant will primarily work at DMH but we do provide anaesthetic cover for elective surgical lists at the elective surgical hub based at Bishop Aukland Hospital. The anaesthetic departments culture is one of mutual respect with collective responsibility for the workload within the hospital. At DMH site surgery is provided within 11 operating theatres covering Community Dental, ENT (Day case), General Surgery (including major colorectal surgery, breast surgery and bariatric surgery), Orthopaedic and Trauma Surgery, Obstetric and Gynaecological Surgery, Ophthalmology and Urology (Day case). Plastic surgery team is based at University Hospital North Durham. Regional Anaesthesia is well established and supported by a regional fellow post. Emergency lists are provided in routine hours for Orthopaedic trauma, General surgery and Gynaecology, as well as an out of hours emergency theatre. There is a Consultant-led pre-assessment service with weekly clinics including CPEX testing.Anaesthesia for CT scanning and MRI services is available. Monday to Thursday we have a PACU to provide Level 1 care of elective patients under the support of the surgical and anaesthetic teams. There is an obstetric department, located on the 6th floor has its own theatre with dedicated elective lists in the main theatre suite. An anaesthetic nurse provides assistance for all theatre cases. The medical department consisits of in house acute internal medicine, cardiology (including a 4 bedded monitored CCU), respiratory (including 6 bedded ARU), haematology, endocrinology, gastroenterology and care of the elderly. Outpatient dermatology, rheumatology and psychiatry will provide virtual and face to face consultations. Palliative care provide face to face support with a team of consutlant and specialist nurses. DMH intenisve care unit admits approximately 580 patients per year with a mix of emergency surgical and medical take. We have 11 funded bed spaces combining level 3 and level 2 care in a unit located near the theatre suite. The consultant rota provides a supoprtive and continuous approach to patient management. The unit is in the process of becoming fully electronic with its documentation. Consultant led RaCI plus nurse led IV, renal outreach and Acute Intervention Teams (critical care inreach) support our work. Weekend work is completed as part of the out of hours commitment with 12 hour shifts. The majority of specialist support is provided via in house referrals with otherwise good relations with our tertiary referral hospitals. Renal and neurology specialities based at James Cook University Hospital provide in house reviews. Our consultant group form a supportive team and recent appointments include a job share with James Cook Hospital and with Acute Internal Medicine. Trust wide collaboration between the intensive care units and specialist support within the trust occurs with regular Critical Care Delivery Group meetings involving the multidisciplinery team from both Durham and Darlington sites. Trust wide ICM specifc a teaching programme is in place with availability of on site simulation equipment. At the weekend there are 3 dedicated consultants who seperately cover critical care, emergency theatres and orthopaedic. trauma lists Consultants and Speciality Doctors are actively encouraged to support the weekly DMH teaching programme and attend our regualar M&M and clinical governance sessions. MAIN DUTIES AND RESPONSIBILITIES The post holder will undertake clinical work in conjunction with their clinical colleagues (24 Consultants, 9 specialty / specialist doctors and an average of 13 trainees) and other professional staff in order to maintain and develop both the intensive care and anaesthetic service and be encourage to develop specialist skills in accordance with the needs of the department and progress to hold departmental non clinical roles. The post holder will have a continuing responsibility for the care of patients in his/her charge and will contribute to the proper functioning of the department of Anaesthesia and Intensive Care Medicine. Furthermore, the post holder will be required to undertake administrative duties directly associated with the care of patients. The post holder will supervise junior medical staff and have responsibility to ensure continuing patient care at all times. For management purposes, the post holder will be accountable to the Lead Clinician for ensuring the effective and efficient use of resources allocated. The post holder will have scheduled activity with regular sessions, fixed continuity weeks and some flexible sessions. SPA activity will be timetabled within the weekly job plan but can be completed flexibily and diarised for review. During some working weeks, flexibility in the ratio of DCC:SPA may be required although SPA time will always be paid back. As additional responsibilities are acquired, the number of SPAs may alter with remuneration agreed through the job planning process. The post holder must possess a UK driving license and be able to present for scheduled list start times as required by the Trust. The post holder will be expected to participate in teaching. Research and clinical audit activities, which are considered to be an integral part of normal duties, will be encouraged. Research funding may be available following successful application to local committees. The post holder will be encouraged and supported to develop services, courses and to undertake innovation. The post holder will be expected to deputise in the absence of colleagues. The post-holder will be expected to perform additional duties in emergency and unforeseen circumstances as are essential for continuity of patient care. The post holder will be responsible for the professional management and supervision of junior medical staff. The post holder will be expected to receive training and participate in educational and clinical supervision. The post holder will be expected to participate in clinical governance arrangements in line with national and local guidelines. If you are involved in the care of a patient who subsequently initiates a claim against the Trust you will be expected to provide, free of charge, an opinion and statement of facts about the case. PROPOSED TIMETABLE AND JOB PLAN This is an indicative job plan, the final job plan/on-call arrangements will be agreed in partnership prior to appointment. Each session is 5.25 hours. Remuneration is 1.313 PAs per working session (which includes pre and post ward round care).
