Care coordinators could potentially provide time, capacity and expertise to support people in preparing for or following-up clinical conversations they have with primary care professionals to enable them to be actively involved in managing their care and supported to make choices that are right for them. Their aim is to help people improve their quality of life. The successful candidate will be based in a local cluster of General Practices as part of the Primary Care Network (PCN). They will be caring, dedicated, reliable and person-focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisational and time management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support. My son loves the outdoors, soft plays areas, trampoline parks, accessing sensory activities or venues, going to arcades in the warm weather and being physically active. He particularly enjoys swimming and water play My son requires one-to-one attention and close supervision due to his inability to understand risk and can be known to self-harm when frustrated or upset, banging his head on the wall or biting himself. He is unable to assess risk or understand consequences to his actions. He generally manages his personal care with prompt and encouragement from others, however, at times may require personal care to assist him in wiping and maintaining his personal hygiene when out and about. My son’s behavior can be challenging at times whereby he may scratch or kick out when he becomes frustrated.
