About the Role: We have an exciting opportunity at The Hospital Social Work Team for a Grade 7 Care Coordinator position (37 hours). The Hospital Social Work Team receives referrals for adult’s aged over 18 to facilitate a safe transfer of care from hospital. The team then completes Care Act Assessment's in the community to assess long-term care needs, focusing on maximising their independence and promoting their own personal goals. You must enjoy working in a fast-paced environment and working closely with our multi-disciplinary system partners. Your main responsibilities as a Grade 7 Care Coordinator: You will manage a varied caseload, which will include completing Interim Support Plans to facilitate safe discharges from hospital, Care Act assessments within the community, and being part of a duty rota. You will deliver exceptional social care to the adults we are supporting and their carers and promote choice and control as much as possible. The team is looking to appoint an individual who is enthusiastic to engage in adopting strengths based, person centred assessments. You will engage in outcomes focused and preventative conversations which build on the strengths of people and their local networks.

Fife Council will offer training and development to meet your SSSC registration requirements and will cover the cost of your annual SSSC registration.
