Your new role Southend City Council are looking to appoint a CHC Lead (Qualified Social Worker) within a 12-month fixed term contract.The successful candidate will have in-depth knowledge and expertise in NHS Continuing Healthcare (CHC) processes and frameworks. They will be required to attend and actively participate in Decision Support Tool (DST) meetings, ensuring robust representation of social care perspectives.
