Other responsibilities will include: Lead the person-centred care planning process, including implementation, monitoring and monthly evaluation. Monitor residents’ wellbeing, recognising changes that impact care and ensuring care plans are updated accordingly. Act as a key lead for safeguarding within residential care, ensuring concerns are identified, escalated and managed appropriately Act as a key point of contact for residents, relatives and visitors, responding to reasonable requests and escalating concerns appropriately.
